Medical Coverage

Choice of three top-tier health plans through CareFirst/BlueCross offering full coverage of routine check-ups, preventative treatment, immunizations, and hospitalization.

HMO
This plan provides the cost savings of an HMO and the flexibility to choose a provider from a network of over 2,600 primary care physicians (PCPs). The PCP will coordinate routine care. If more specialized care is required, the PCP will refer to a participating specialist in the network. There is no coverage out of network except in the case of an emergency.

POS
This plan provides the cost savings of an HMO and the flexibility to choose a provider from a network of over 2,600 primary care physicians (PCPs). No referrals are required to visit a participating specialist. There are out of network benefits should you elect to see an non-participating doctor.

PPO
This plan offers members the flexibility to choose their own doctor or hospital and control health care costs. Members save money when they obtain their health care from the large PPO network. This includes a national PPO network throughout the US and overseas.

Prescription Drug Coverage

Drug Program included with all health plans. Benefits include:

  • Retail program allows for 34 day supply of medication
  • Mail Order program
  • Maintenance Drug program
  • Use your prescription drug card at more than 59,000 participating pharmacies nationwide.

Dental Coverage

Comprehensive Indemnity dental coverage that allows you the complete freedom to see any dentist you choose. The plan also features:

  • 100% Coverage for preventive and diagnostic services
  • 80% Coverage for Basic and Surgical services
  • 50% Coverage for Major Restorative services
  • Orthodontia coverage for dependents under age 19.

Vision Coverage

Professional vision services including routine eye examinations, eyeglasses and contact lenses by national network of providers.

  • Routine eye exams covered once per year
  • Discounts on eyeglass frames, lenses or contact lenses
  • Laser Vision Correction Services at significant discounts through a network of experienced, credentialed surgeons.

Flexible Spending Account (FSA)

A Flexible Spending Account, also known as an FSA, allows you to set aside money, on a pre-tax basis, to pay for certain expenses. Because this money is deducted from your paycheck before taxes are taken out, the total amount of income on which you pay taxes is reduced.

Health care FSAs may be used to pay for many out-of-pocket health care expenses such as eyeglasses, dental care, prescriptions, hearing aids, deductibles, or co-pays. With a dependent care FSA option, employees may pay for certain dependent care expenses, such as day care, before/after-school care, and preschool (not kindergarten).

Short Term Disability (STD)

INNOVIM provides a STD benefit of 60% income replacement of weekly salary up to 13 weeks. Benefits begin on the 1st day for an accident and or the 8th day for sickness/illness.

Long Term Disability (LTD)

INNOVIM provides a LTD benefit of up to 60% income replacement of monthly salary. Benefits will be paid for the duration of the disability. Benefit reductions will occur at age 65.

Life Insurance and Accidental Death & Dismemberment (AD&D)

INNOVIM provides at no cost, a basic Life/AD&D benefit of $50,000 per employee.

Additional Voluntary Life Insurance can be purchased for yourself, your spouse and dependents.

Employee Assistance Program (EAP)

EAP offers confidential, personal counseling (marriage/relationships, stress, grief, anxiety, legal, financial, child and elder care referrals, etc.)

Supplemental Medical Insurance

Employees can elect coverage through AFLAC, choosing from a variety of plans, such as Accident, Personal Sickness and Critical Illness.
